Seton Medical Center Williamson is proud to announce the Fall Sports Injury Clinic to support young, injured athletes grades 7-12 in our community. The clinic offers comprehensive services, from a FREE assessment by an orthopaedic specialist to on-site imaging and rehabilitation services—with no appointment needed.

CLINIC PHYSICIAN

Erik J. Bruce, MD  Board certified in Orthopaedic Surgery; Fellowship trained in Sports Medicine (4-yr football letterman, Tulane University, New Orleans, LA; Captain, senior year)

Registration Process:

Upon arrival in the Registration department, you will be asked to verify all demographic and insurance information. You will be required provide the following information:

• Health insurance cards and information including address, telephone number, and policy number
• Photographic identification card
• Social Security card
• Referral or authorization numbers, if required by your insurance

You will also be required to sign the Conditions and Consent for Treatment form at the time of registration. If you are a minor an adult (over age of 18 years old) will be required to sign all necessary documentation.

 During your screening with Dr. Bruce, if additional services are required: 

(a)    X-rays will be available on site. 

(b)    If an MRI is needed, you will be asked to schedule during the following weekday or directed to the emergency department.

(c) If non emergent surgical services are needed, Dr. Bruce will work with you to coordinate a schedule or you may make your own arrangements for surgery with another physician of your selection. Our Emergency Department is available for any emergent needs.

THESE ADDITIONAL SERVICES WILL BE BILLED DIRECTLY TO YOU OR YOUR INSURANCE PROVIDER.
